Press Release

At approximately 5:00 PM the
Logan 911 Center received calls reporting that a motorcycle /vehicle crash had
occurred at approximately 2800 North Main in North Logan. At the scene it was determined that 53 year
old Dwayne Monson of Preston (date of birth 10-21-1960) was north bound on Main
Street riding a Honda Shadow Motorcycle.
Traffic was slowing for a construction zone to the north. A black Silverado pick-up, driven by
Kaylene Wootton of Providence was coming to a stop in the inside north bound
lane due to traffic merging into one lane for the construction zone. Mr. Monson’s motorcycle struck the left
rear bumper of the pick-up truck ,just before the truck came to a stop. The motorcycle and its driver were thrown through
the air and came to rest in the south bound lanes of travel. Mr. Monson was wearing a helmet. He was transported to the Cache
Valley Hospital and was later taken by helicopter to a Salt Lake City
area hospital. He was listed in very critical condition.

Press Release
Vehicle #2 was a 2004 GMC Sierra Pickup Truck bearing Utah lic. # UTZ2369VV. It was driven by a Jared Astle of North Logan. Mr. Astle’s is 28 years of age. He was traveling south bound on the highway in the inside lane.
Vehicle #3 was a silver 2004 Honda Civic driven by Vicente Mejia. Vehicle #3 was headed north bound in the inside lane of travel. Vehicle #3 was struck by debris from the collision of Number #1 and #2.
It appears from our investigation that the driver of Vehicle #1 attempted to make a left hand turn to go north bound. Traffic for south bound vehicles was heavy. Vehicle #2 was south bound in the inside lane of travel. When vehicle #1 pulled out there was no time for vehicle #2 to avoid the collision or stop. Impact occurred on the driver side door of the Mazda Pickup. Passersby attempted to preform CPR on the Mr. Frew. He was transported to Cache Valley Hospital where he was pronounced dead shortly after 12:00 PM. The driver of Vehicle #2, Mr. Astle, was transported to Logan Regional Hospital with complaint of neck pain. He was being evaluated at the time of this press release.

Cache Valley Hospital Gunman Arrested
Jason James Burr has been taken into custody upon his release from the University of Utah Hospital on today's date, June 6, 2014. North Park Police Investigators transported Mr. Burr to the Cache County Jail. Mr. Burr has been booked into jail on the the following charges:
Attempted Aggravated Robbery, a second degree felony
3 Counts Aggravated Assault, third degree felonies
2 Counts Possession Dangerous Weapon by Restricted Person, third degree felonies
2 Counts Carrying a Dangerous Concealed Weapon, A misdemeanors

Update Cache Valley Hospital Shooting 05/19/2014
Our investigation has revealed that Mr. Burr did come to the Cache Valley Hospital seeking prescription pain medications.
Mr. Burr was armed with two hand guns both of which were loaded and ready to fire. Mr. Burr raised and pointed both guns at hospital staff in an aggressive manner while making his demands. One gun was a Taurus 9MM semi-automatic and the other was a .38 Special two shot Derringer style handgun.

Patient Update on Cache Valley Hospital Shooting
Mr. Burr has come out of surgery, and has condition has been listed as critical.
Suspect in Cache Valley Hospital Shooting Identified
The suspect involved in the Cache Valley Hospital shooting has been identified as 34 year old Jason James Burr from North Logan Utah.

At approximately 8:20 am on Friday May 16, 2014 North Park Police were dispatched to Cache Valley Hospital emergency room located at 2380 N 400 E, North Logan on a report of a single active gunman. North Park Police, along with assisting agencies from Logan City, Cache County Sheriff, Smithfield Police, Utah Highway Patrol and USU Police, responded and subdued the suspect.
Suspect was shot 4 times and is in critical but stable condition. Individual is in the process of being transferred to a Salt Lake Hospital.
Cache Valley Hospital and Greenville Elementary were placed on lockdown. No other injuries were reported.
Hospital is now open, but the emergency room is on divert.
A press conference will be held at North Park Police Department at noon on Friday May 16, 2014. Address for North Park Police is 575 E 2500 N, North Logan.
